Need jack board

Hi. I need the part number for the yellow 3.5 jack thats under the cover on the side of the cameras that you hook the audio and video output cable to. That jack accepts the special 4 conductor 3.5 plug on the cable ( standard camcorder cable)itself so on the jack board it uses a female 3.5 jack. That 3.5 mm yellow chassis mount jack is on a tiny board so it comes as one piece. The board and the yellow jack.I need the part number for that jack board/jack assy. The yellow female jack is broken. Thanks alot!
